 Please try "make GEN_INDEX=yes" for documents whose Makefile has
 HAS_INDEX=yes.  GEN_INDEX is used to generate an index.  For handbook
 it can be found at http://www.freebsd.org/doc/en_US.ISO8859-1/books/handbook/doc-index.html.

 It is always set as "true" on www.FreeBSD.org but the default value
 is "false" because of the large memory footprint (~100MB in the case
 of the handbook).  Nowadays it is no longer a big deal for x86
 platforms, though.
