I've tried all framerates (5-30) for sizes sqcif, qsif, qcif, sif, cif, vga.  
All failed.  

Strangely pwcview is reporting (at all framerates):
Webcam set to: 352x288 (vga) at ?? fps

Isn't that supposed to be 640x480?

sqcif fails with (at all framerates):
Webcam set to: 176x144 (sqcif) at ?? fps
libv4l2: error converting / decoding frame data: v4l-convert: error 
destination buffer too small (18432 < 38016)
Error reading from webcam: Bad address

webcamd did report some errors:
gspca: frame overflow 28838 > 28672
