kernel trap 12 with interrupts disabled

I am getting a really bad crashing issue on a few of my servers after they
have been running great for almost 1 year now. The main error is "kernel
trap 12 with interrupts disabled" but here is a screen dump from the crash.

The server hangs from there and has to be hard reset with power button.
