"terminated ioc" means that the HBA decided to terminate the command.
"804b" is an LSI internal code.  Steve (CC'd) might be able to make sense
of it.  I doubt there's anything wrong with your cabling, but if a power
cycle fixed the problem, you might've had a firmware crash in the HDD, HBA,
or expander.
