iscsi_initiator and tag opening problem

yeh, but 'it works with MS' :-(

the first one with the problem was iscilon, but today, something similar
i'm seeing with IBM storewiz.

it's a CAM thing not PDU/BHS, or at least that is my undertanding.
myabe it's time to read the RFC again
