iscsi_initiator patches

hi,
though I'm the developer of iscsi_initiator,  I don't have (nor want)
a commit bit, so could someone with such privilige please apply it?
The current distributed version does not realy work.
this new version has:
	- support for header/data digest
	- no limit on targets (actually 64, but it's a compile option)
	- some deadlocks fixed.
	- a memory leak fix
