decoding mpt driver messages

Howdy,
Can anyone step through this series of messages from a Dell/LSI RAID card
for me? Since there's no management access from within the OS and the
server is remote, I'd like to know if this is truly a drive failure or if
it's like the old Adaptec cards that would just sometimes "lose" a drive.

Also is there any way to talk to the drive with smartmontools? Any way to
get a serial number off the drive to check warranty status before I visit
the machine?
