Also check for possible settings in other mixers (you probably
have more than one):
% mixer -f /dev/mixer0
% mixer -f /dev/mixer1
% mixer -f /dev/mixer2
...
Maybe the noise you're hearing is some amplified "interference"
coming through a monitor channel; look for "monitor", "igain",
"ogain". With a graphical mixer like xmixer or aumix-gtk, you
can easily check the effects of those settings.
