Adding custom package to installation.

Hi. I'm attempting to add a custom built package to
the 4.10 Release, and have an error:

"Unable to fetch pakcage dan-1.0.0 from selected
media. No pakcage add will be done."

I would like to have this package be added at
installation time (using install.cfg).

The setup:

1. Built a package with pkg_create. It installs fine
with pkg_add.

2. Added the package (dan-1.0.0.tgz) to packages/All

3. Added this entry to packages/INDEX :

dan-1.0.0||/|Test package||mrdan at mail.com||||

Also, what are the fields of the INDEX file? Thank you
for your help.
