portupgrade has a config file (/usr/local/etc/pkgtools.conf), which offers 
many config options for you to tweak - including the ability to specify 
ports to ignore - look for the section about HOLD_PKGS.

Ports and packages keep their install data in the same place, yes -
/var/db/pkg.

Have you read man sysinstall?  But bear in mind that "make buildworld" was
carefully designed to upgrade the system properly.

The -P or -PP options to portupgrade should work, but read the manpage for
caveats.
