Update all of the RT Extension ports to modernize and flavourize them.  Now
there will be rt44 and rt42 flavours built and available in the default pkg
repositories by default.  (Where appropriate: several of these modules are rt42
only.)

Drop all references to rt40, which has been removed from ports.
