net-mgmt/prometheus-collectd-exporter: Update and add rc script

This diff updates collectd_exporter to 0.4.0 and adds an rc script.

Although the `PORTNAME` is `prometheus-collectd-exporter`, I've opted to keep
in-line with other ported exporters and the rc script and variables all refer
to `collectd_exporter`.

It may be that the `PORTNAME` should be changed to `collectd_exporter` too, to
be inline with the other exporters.
