Two names when creating a module

Hello everyone,
                  While I'm testing with creating modules in FreeBSD , I
encountered giving two names for a module once in module_t structure and
the other when using DECLARE_MODULE(name,module_t struct).Why cant we give
a single name .Why are there two names when we create module.I read the man
page of FreeBSD but it didnot explain much .
