better way than hacking termcap to be ca_mode free

Not sure about ~/.termcap but you can just override ti/te via TERMCAP in
environment.

  $ export TERM=${TERM:-xterm}
  $ export TERMCAP=${TERM}:ti@:te@:tc=${TERM}:

I guess the diff is not for termcap file from /head.
