Looks to me like it's trying to access swap and failing due to the
ad0 errors. The panic is a result of this failure.

The atapi drivers are pretty flakey at the moment :-( I've been forced
to run my drive in PIO4 mode to avoid the DMA errors! Of course, your
disk might also be failing.
