  o  Make debugging code conditional upon KDB instead of DDB.
  o  s/WITNESS_DDB/WITNESS_KDB/g
  o  s/witness_ddb/witness_kdb/g
  o  Rename the debug.witness_ddb sysctl to debug.witness_kdb.
  o  Call kdb_backtrace() instead of backtrace().
  o  Call kdb_enter() instead Debugger().
  o  Assert kdb_active instead of db_active.
