Do you have multiple pkg repositories on this system?  I've been seeing 
this behavior to happen for years when more than one repository is 
configured.

We always had 2 repositories on pfSense and it was not something trivial 
to see on FreeBSD systems, but nowadays with pkgbase it's becoming more 
common so I guess more people will see it happening.
