The /usr/local/etc/rc.d/couchdb3 file has an error in the path in the start
section

This

HOME=/var/run/couchdb \
ERL_CRASH_DUMP=/var/run/couchdb/erl_crash.dump \

must be:

HOME=/var/run/couchdb3 \
ERL_CRASH_DUMP=/var/run/couchdb3/erl_crash.dump \


The complete start section is:
