PDF with filled form fields

cups-filters 1.28.15_5 and also 1.28.16_1 pdftopdf is breaking PDF files with
fillable forms.  Text entered in some fields which is visible when edited with
qpdfview and displayed with mupdf does not display or print after processing by
pdftopdf.

By way of example, I am attaching an IRS Form 941 pdf file.  I have entered
12-3456789 as the EIN number at the top and a Test Company name and address. 
The first file is as saved by qpdfview.  Displaying with mupdf shows the filled
in fields.  The second file is after processing by pdftopdf.  Displaying this
with mupdf shows blank fields.

pdffonts lists the same set of fonts for both files, albeit with different
object numbers.  What other info can I provide?
