I also found that vacuum does not shrink the db file much. But it does remove
the journal for me.

NB: Your files in /var/db/pkg are from Feb, 9th. That seems relatively old.
Just noticed that pkg nowadays stores the files in /var/db/pkg/repos/FreeBSD.

On my machine:
# ls -l /var/db/pkg/repos/FreeBSD/
total 60520
-rw-r--r--  1 root wheel 61919232 Jun 29 21:19 db
-rw-r--r--  1 root wheel        0 Jul  3 09:21 db-journal
-rw-r--r--  1 root wheel        2 Jul  3 09:21 groups.ucl
-rw-r--r--  1 root wheel        0 Jul 19 09:21 lock
-rw-r--r--  1 root wheel      172 Jun 29 21:19 meta

vs

# ls -l /var/db/pkg/repo*sqli*
-rw-r--r--  1 root wheel 61403136 Apr 26 04:10 /var/db/pkg/repo-FreeBSD.sqlite
-rw-r--r--  1 root wheel        0 Apr 28 18:21
/var/db/pkg/repo-FreeBSD.sqlite-journal
