Looking again at your fdisk output, I suspect it contains some junk for slices
2, 3 & 4 so FreeBSD is ignoring it.  That is why there is no gpart output or
/dev nodes for the slices.

Try booting in verbose mode (via the boot options menu) and then look in the
dmesg output for GEOM_PART messages.
