Patch

PulseAudio enables module-device-restore and module-card-restore, which load
and store device and card volume settings, by default. On FreeBSD, this
functionality is already handled by rc.d/mixer. Further, if PulseAudio is not
enabled on a system-wide basis, PulseAudio restores its own mixer settings when
an application needs it.

As a result, module-device-restore and module-card-restore cause a jarring
experience on FreeBSD. For example, playing a video in Firefox sometimes causes
the mixer volume to change suddenly as PulseAudio loads what it thinks were the
previous mixer settings.

This patch changes the example default.pa to not load these modules by default.
This has no affect on systems with PulseAudio already installed and configured.
