I had missed that the filesystem had been mounted with the 'sync' option. I
will have to check, but off the top of my head that will probably disable soft
updates. If so it is very surprising that there can be any failures at all. I
will investigate and let you know. Will be next week before I am likely have
time to do the investigation though.

As for the other timings, they are irrelevant if soft updates are disabled.
