If the LDAP (and other extensions) can be build without the main stuff 
of dovecot (a theoretical "configure 
--only-ldap-extension-but-not-dovecot-itself && make", then a 
master/slave port combo would be the way to go.

If it can not not be build without the main part of dovecot, and if it 
doesn't change anything else in the parts which are build without the 
LDAP option set, the experimental subpackages support would be the way 
to go (please see the thread on freebsd-ports@ for the issues/bugs some 
people (me included) see with the current implementation of subpackages, 
I have not evaluated if the issues I see would affect dovecot or not).
